Local Number Portability (LNP) allows you to move your current telephone numbers from your existing carrier to eSmith IT Consulting, Inc. (“eSmith IT”). Federal Communications Commission (FCC) regulations require this signed Letter of Authorization as legal proof that you have explicitly requested and authorized the transfer of your telephone number(s).

CRITICAL PORTING RULES & SERVICE CONTINUITY:

1. DO NOT CANCEL YOUR EXISTING PHONE SERVICE: Your account with your current provider must remain active and in good standing until the port is completely finished and confirmed by eSmith IT. If you cancel your service prior to port completion, your current carrier may release your telephone numbers into the general pool, resulting in permanent loss of your phone numbers.

2. ANCILLARY SERVICES WARNING: If you have DSL internet, distinctive rings, security alarms, elevator lines, POS terminals, or fax lines tied to your Billing Telephone Number (BTN), porting may cause automatic cancellation or disruption of those services. Contact eSmith IT before porting if your internet is bundled with your phone line.

3. NO SERVICE CHANGES DURING PORT: Do not submit any address changes, feature modifications, or disconnect orders to your current carrier while a port request is pending, as this will immediately freeze and reject the port order.

Required Proof of Ownership (Phone Bill Submission)

Losing carriers reject all port requests that do not include a matching Customer Service Record (CSR) or recent phone bill. Please email a copy of your most recent telephone bill (dated within the last 30 days) to:

By completing and executing this document, I designate eSmith IT Consulting, Inc. and its designated underlying telecommunications carriers/CLEC partners as my authorized agent to act on my behalf to:

Transfer the telephone number(s) listed above from my current telephone provider to eSmith IT;
Obtain Customer Service Records (CSRs), billing information, and technical provisioning data from my current service provider;
Coordinate local number portability (LNP) and toll-free portability (TFNP) with the national numbering database and relevant telecommunications entities.

I affirm that I am at least 18 years of age and legally authorized to make carrier changes for the telephone numbers listed herein. I understand that only one telecommunications provider may be designated for a specific telephone number. I agree that eSmith IT may consult with my current carrier regarding applicable fees or disconnection intervals, and I agree to all terms and conditions stated in the eSmith IT Service Agreement.

Designation of Authorized Agent: By checking this box, I designate eSmith IT Consulting, Inc. and its upstream carrier partners as my authorized agent to execute this port request, obtain CSRs, and transfer the listed telephone numbers. I affirm that I am an authorized representative with full corporate authority to execute this LOA.
